cOMMUNITY sCHOOL COLLABORATIVE

Program Overview

The Community School Collaborative (CSC) is a joint project of Guadalupe Alternative Programs (GAP) and Change Inc., providing prevention, early intervention and mental health services for at-risk K-12 students in select metro area schools.

Our School-Based Mental Health Services are provided at school during school hours, in Out-of-School Time in the community, and in the home. School-based mental health delivery is a national movement with strong support in Minnesota. Schools, school districts, and government agencies all contribute to funding these efforts (in addition to insurance reimbursements).

With this service model, we also provide staff development to equip teachers and other staff members with methods to be more trauma-informed in their work with students. We also work with parents and support their efforts to provide nurturing home environments.

CSC provides the following effective services to meet the unique needs of each student:
Targeted case management
Conflict mediation
Youth development and skill building
1:1 mentoring
Parenting education and support
Diagnostic assessment
Treatment planning
Individual, group and family therapy
Teacher and parent consultation
​Staff training and development

FUNDING

Minneapolis Public Schools, Saint Paul Public Schools, Hennepin County, and the Minnesota Department of Human Services primarily fund CSC programming. Mental health services are also billed to insurance when appropriate. All students are eligible for services regardless of legal status or type of insurance coverage. We also pursue private funding to support program growth and to ensure at-risk students have access to critical mental health services.
