Change Inc.'s Partnership Program for Youth and Family Wellbeing
The Partnership Program for youth & family wellbeing is for African American students who cannot attend school due to mental health, social, emotional or behavioral issues. It is a voluntary program that helps students get back in a school setting and experience success.
The service model:
A student and their family will have one or more family partners. The family partner helps the family identify their concerns, needs, hopes, dreams, strengths, and vision for the future. The partner then helps the family form a team that will support them in developing and executing a plan to achieve their goals. Team members can include relatives, friends, faith and community members, trusted professionals, and other supportive people in their life. The Partnership Program values family voice and choice. It focuses on the things the student and family believe are important for success.

The student must:
- identify as African American
- be in kindergarten through 12th grade
- live in Ramsey County
- be experiencing academic crisis
- has a diagnosis of, or symptoms that indicate a mental, behavioral, substance abuse, mood, anxiety, development, conduct, emotional, or scholastic skills disorders
- be at risk for out-of-home placement or returning from an out-of-home placement
- CASII or Functional Assessment Score of four or higher
- have been served by at least two of the following: special education/ IEP or 504 Plan, mental health provider, substance abuse provider, medical provider, development disabilities provider, juvenile services/court, child protection, or family court.
- have had at least three services from among the following types: mental health case management, mental health therapy, partial or full hospitalization for mental health, psychiatric residential treatment, children's therapeutic services and supports, chemical dependency treatment, eating disorder treatment, medication management, residential treatment, school interventions, day treatment, or juvenile detention.
